  • Document the plan in writing

Monitoring:

  • Monitor and evaluate how employee is progressing
  • Take corrective action or make changes to the plan

Reviewing:

  • Annual performance review and evaluation
  • Sign off and reward (if applicable)
  • Begin new cycle
